Our Clinical Team is a group of caring and passionate professionals dedicated to improving the lives of our most vulnerable members. Our team includes physicians, nurses, social workers, medical assistants, behavioral health specialists and others who provide in-home primary care and transitional care services. A goal for this program is to improve our offering of palliative and supportive care services for members with advanced illness.

The Certified Medical Assistant (CMA) is an integral part of the Patient Care Team (PCT) and closely collaborates with the Provider inside and outside of members' homes. Together, the CMA and Provider work closely to ensure that care in the home is optimally provided and that follow-up items are appropriately executed. The CMA is under the direct supervision of the Provider at all times and performs his/her duties in accordance with licensure, privileges, and scope of practice regulations.

This is a field-based position that requires driving to and conducting visits in patient homes. We are seeking Medical Assistants (CMAs) in Atlantic City - New Jersey

As a Certified Medical Assistant, you will:

  • Assist the Provider as directed in all facets of patient care before, during, and after the in-home visit as well as post-visit office duties as directed.
  • Interact directly with the patient during the in-home visit, including but not limited to:
    • Translate or language line connection for the patient, caregiver, family members, and/or Provider.
    • Collect and document demographic information, care preference information, medication-related information, environmental information, vital signs, and other information as directed by the Provider.
    • Perform patient blood-laboratory draws as directed by the Provider.
    • Conduct member outreach to schedule home visits as needed for Clover clinical programs.
    • Review required consent forms with the patient and obtain the patient's signatures on required consent forms (General Consent for Treatment, HIPAA Notice of Privacy Practices, etc.) during the initial, and if necessary, subsequent in-home visits.
  • Assist the Provider with the closure of open care gaps and other priority items for in-home visits.
  • Coordinate care with other PCT members, including through EHR task-management software.
  • Maintain medical supplies for CMA and Provider as prescribed by practice standards.
  • The CMA is to remain with the Provider at all times during the visit.
  • Participate in ongoing professional development and self-improvement.
  • Demonstrate flexibility in assignments within Clover’s Clinical Programs based on member and company needs.
  • Complete tasks as directed by Clover Health Provider, Operations and Clinical leadership teams.
